Enrollment Agreement - American Firearm Academy

AMERICAN FIREARMS ACADEMY, HEREBY CALLED “THE COMPANY”, IS AN EDUCATIONAL REFERRAL PROGRAM PROVIDING HIGH QUALITY TRAINING TO INDIVIDUALS LOOKING TO PURSUE A CAREER IN AN EMERGING MARKET THAT HAS HIGH DEMAND FOR QUALIFIED PROFESSIONALS. THE TRAINING COURSES WE OFFER ARE CONDUCTED BY STATE LICENSED INSTRUCTORS WITH YEARS OF EXPERIENCE IN THE APPROPRIATE FIELD. THE COMPANY AND BELOW LISTED STUDENT ENTER INTO THIS AGREEMENT UNDER WHICH THE STUDENT WILL PAY ALL TUITION AND FEES AS DESCRIBED AND ADHERE TO ALL POLICIES OF THE COMPANY. THE COMPANY WILL PROVIDE THE AGREED UPON SERVICES AS LISTED AND IN ACCORDANCE WITH THIS AGREEMENT. (POLICIES ON REVERSE SIDE)

I HAVE HAD ALL OF MY QUESTIONS AND CONCERNS ANSWERED TO MY SATISFACTION AND I AGREE TO ALL STATED POLICIES AS SET FORTH IN THIS AGREEMENT. I ACKNOWLEDGE THAT I HAVE RECEIVED A COPY OF THE AGREEMENT. I UNDERSTAND THAT IN ORDER TO RECEIVE ANY SERVICES AND CERTIFICATIONS THAT I MUST MEET ALL OF THE REQUIREMENTS OF THE COMPANY AND THIS AGREEMENT. RULES, REGULATIONS, TERMS, CONDITIONS & POLICIES (READ CAREFULLY & CHECK ALL)

Payments(Required)

Presently payments are accepted in credit/debit card or money order only, personal or business checks are not accepted. There will be processing fees added to all credit/debit card transactions. Remittance of any payment constitutes full agreement with all terms, conditions, policies and regulations of the agreement.

Courses(Required)

All students must present a company training ticket in order to attend any course. All students must arrive for their scheduled class no later than 15 minutes before the scheduled start time. Any students arriving more than 15 minutes after the scheduled start time, will not be allowed to attend the course and will have to reschedule for another date/time. Any student not in class on their scheduled date is subjected to a $25 fee to reschedule the course (no exceptions). Students must adhere to the rules and regulations of the training facility for the course(s) that are being conducted.

Certificate or license of completion(Required)

All certificates of completion will be received by the students only when all of the below requirements are met. 100 attendance in course(s) Passing of exam All balances are paid in full

Licenses(Required)

Where applicable, any state, federal or local government license will be sent directly to the applicant. The company is not responsible and not to be held liable for any applicant’s license application being denied by the issuing agency. All reasons for denied applications may not be known to the company but may be known to the applicant. The company will not issue any refunds due to the applicant being denied of any licenses.

Other(Required)

Proper attire must be worn at all times while in any company facility. Violence, vulgarity, or harassment of any type is prohibited. Use of alcohol, drugs, or any other illegal substances are prohibited from this facility. The company reserves the right to eject or ban admission and any person from any company facility. If the circumstances require action, the company will notify the proper authorities and/or pursue legal action. The company reserves the right to cancel or reschedule any course(s) without prior notice. If a course(s) is canceled, it will be rescheduled at the company’s discretion. Further, the company reserves the right to charge training venue as needed. Free resume writing and interview tips assistance are available to all students if requested.

Refunds(Required)

There are no refunds. All payments are final. At the time of enrollment, you will be given a course schedule and a seat will be reserved for you in the scheduled class. There are no refunds if any required license(s) are denied or if you simply change your mind. Also, if you provide false information of any kind or do not divulge any existing criminal history, there are no refunds of any denied payment.

Placement assistance(Required)

The company offers free lifetime job placement assistance to all students. Placement assistance is provided to all students that have met all requirements of the agreement. There is no obligation on behalf of the students to participate. This service does not guarantee employment, pay rate, start date, job site, uniforms, or benefits. This service may be terminated by the company any time or if you are dismissed due to violence, theft, or harassment of any kind.
